Design dispensation request form

  • Use this dispensation form to request a departure from compliance with Greater Western Water's applicable design standards.
  • A design dispensation request is to be submitted by the key design personnel (checker).
  • Please include a Greater Western Water Reference Number for Land Development (LND number) where applicable.
  • The organisation name and contact name should be as they appear on the accredited contractors or consultants list.
  • All fields are required unless marked as ‘optional’ or ‘if applicable’.
